Availability of Form 1095-C

In accordance with federal requirements under the Affordable Care Act (ACA), Form 1095-C is available upon request for employees of Onsted Community Schools.

Form 1095-C provides information about the health coverage offered to you by the district during the applicable tax year.

If you would like to receive a copy of your Form 1095-C, you may request one using the contact information below.
